16、生素耐藥機制,改變細胞膜的通透性 使抗生素滲透障礙產生滅活酶和鈍化酶改變抗生素作用靶位,ESBLs Plasmid-Mediated Extended Spectrum Beta-Lactamase,對三代頭孢菌素如頭孢他啶、頭孢曲松、頭孢噻肟或氨曲南的抑菌圈減小(R、I、S)加克拉維酸可使抑菌圈擴大(≥5 mm)如為ESBL,應報告所有青霉素類,頭孢菌素類,氨曲南耐藥,即使體外敏感,也應視為耐藥,ESBLs產生/增

18、BL組(18%),(P=0.06),,抗生素治療過程中誘導產生 并可選擇出持續(xù)高產AmpC突變體第三代頭孢菌素是弱誘導劑,但具有選擇去阻遏突變株作用β-內酰胺酶抑制劑均不能抑制AmpC酶 相反,克拉維酸是強誘導劑突變株不僅對第三代頭孢菌素耐藥,對β-內酰胺類抗生素/酶抑制劑復合物也耐藥碳青霉烯對AmpC酶高度穩(wěn)定,沒有選擇去阻遏突變株作用,I型β-內酰胺酶(AmpC酶),抗生素應用與AmpC突變,抗生素種類
